case 2: dysplastic hip with secondary osteoarthritis with proximal migration and shortening. The acetabulum shows a reasonable size defect which would need grafting for solid implantation of a cementless cup.

Postperative after reconstruction with a bulk femoral head autografting. Graft was held with 2 long screws superiroly. Reaming reshaped the acetabulum to take a good press fit cementless cup augmented with screws. Otherwise reconstruction was stable.

On the lateral views showing both lateral profiles of the femoral shaft as well as the iliac wing, the screws of both graft and cup can be viewed clearly as well as their orientation. One can see the cup to be well supported by the graft.

Follow up after 7 years show the remodelling of the graft and the stable construct.
